Replantation is the procedure of returning the knocked-out tooth to the alveolus after avulsion. It is the therapeutic choice for permanent teeth, and contraindications for its performance are the compromised general condition of the patient and an avulsed primary tooth. The teeth that are most often replanted are the upper front incisors due to their exposed position. Replantation can be immediate or delayed. Immediate involves restoring the tooth immediately at the scene of the accident, while delayed requires storing the tooth in a specific medium in order to extend the time until the replantation procedure itself. Transport media serves to preserve periodontal ligament cells. An ideal transport media has all the necessary nutrients, is antimicrobial, has the appropriate osmolarity and pH. After reimplantation in the alveolus, the tooth is fixed with a flexible composite-wire splint or a titanium trauma splint. Minimal mobility is ensured for optimal healing of the PDL. During avulsion, each tooth is left without blood circulation and consequently pulp necrosis occurs. Endodontic therapy differs for teeth with open and closed apex. PDL healing after replantation depends on factors such as length of extra-alveolar time, mechanical damage, and bacterial contamination.
